RATES
Rates are based upon the type of service provided (ie:assessment, therapy session) and modality (ie: individual, group). Please contact us for more information.
INSURANCE
Many health care insurance companies provide out-of-network benefits for their members. You will be provided with a monthly statement that will contain all of the information you need should you choose to submit it to your carrier for reimbursement.
***It is very important that you check with your health care insurance carrier to determine if your policy includes out-of-network benefits and how much. You should also determine from your health care insurance representative if you have a deductible and whether you have already met the required minimum for your membership year. You should also find out if there is a maximum reimbursement policy (ie: number of sessions per year and reimbursement amount).
PAYMENT
Cash or checks are accepted for payment. In most cases, payment is due at the time of each session (special programs may require a different payment schedule).
CANCELLATION POLICY
It is important that you give at least 24 hours advanced notice if you will not be attending a scheduled appointment. In the event you do not give proper notification you will be billed for the full appointment fee.
